Our staff is available Monday-Friday 9-5. Please let us know at least 24 hours in advance if you have to cancel your appointment. We appreciate your call if you are unable to arrive on time.

Before Your Patient Visit
Obtain a referral from your primary care provider if needed.
Verify that our physicians are members of your insurance plan.
Bring your insurance card.
Select a laboratory within your network.
Complete a Patient Registration Form (required annually).
Be prepared to pay your portion of the service fee which is not covered by your insurance, such as co-pays, co-insurance and deductibles.
New patients complete a “Patient History Form”.
Complete an "OB Prenatal History Questionnaire” if pregnant.

Insurance & Payment Policies
Physicians For Women participates with a number of major insurance plans. Please contact us to verify that we are in your plan’s network. It is important to bring your current insurance card to every visit, and if necessary a primary care physician’s referral. As required by your insurance company, you are responsible for the portion of your charges not covered by your insurance policy. Co-payments, coinsurance and deductibles are due at the time of service. We accept Visa, MasterCard and Discover Card for your convenience. Special financial arrangements can be made for our obstetrical and surgical patients. If you require assistance, please talk with our insurance coordinator or office manager.

Prescriptions and Prescription Refills
Your care is very important to us, and we believe that the best care can be provided when we have seen you in the office.  We will gladly write and refill prescriptions, if appropriate, at the time of your office appointment. If the prescription was written for less than a year supply, you generally need a follow up appointment for continued treatment.

If you need prescriptions written for mail order companies, please tell us that at the time of your visit.  We will provide the prescription so that YOU can mail or fax it in to your mail order prescription service. We will ONLY fax an already completed form that you bring, at the time of your appointment. This helps to ensure that all of the needed information is correct.

WE WILL NOT FILL A FAXED PRESCRIPTION REQUEST FROM YOUR PHARMACY. We do not feel that your pharmacy should be directing your medical care and prescriptions. If you need a prescription refill, we request that you follow up with a visit in the office so that we can give you the time and consideration that you deserve.

Test Results
Follow up appointments may be recommended by your practitioner or physician to discuss your test results. You will not receive a call for normal routine pap smears or other routine blood tests; however, you may have access to your normal routine lab tests on-line. Please ask us about this option during your appointment. Otherwise, you may call our office during regular business hours to confirm the results of these routine tests.
